Required Shots -- Grades 6 to 12

A new state requirement on immunizations goes into effect the first day of the 2010-2011 school year.  Students in all grades 6 through 12 must have these additional shots completed and documentation of these shots must be turned into the school office before August 11, 2010. 

•  A Second Chicken Pox Shot (Or proof that the child had chicken pox can be accepted instead of the shot)
• A Meningitis Shot
• A Dtap Shot
